2019/2020 VARSITY REGISTRATION FORM

The Centennial "Varsity Registration Form” must be fully completed before any student- athlete can participate in a practice. All student-athletes are required to complete this online before being eligible to practice further with the team again. Head Coaches should communicate to prospective players that a Player Registration Form is required before they can participate, and advise that forms are posted on the Athletics website. Computer terminals are accessible in the A&R office, and in the Student Centre Computer Lab after hours. 

All Varsity Registration forms must be downloaded, filled and sent to Joan Healey at jhealey@centennialcollege.ca
